About the practice

We began in a single Waltham office with four nurses and a telephone. A decade on, we coordinate care across two counties — and the working method has not changed. Each plan of care is written with the patient present, reviewed by our Director of Nursing, and revised the moment circumstances shift.

We hold ourselves to the standards our referral partners expect: prompt post-discharge contact, documentation that arrives when promised, and LPNs who know the history before they knock.

Families are not clients to us. They are the second half of the care team, and we brief them accordingly.
